Application for Appearance Review and/or Site Plan Review	This application is used for items which DO NOT need a Public Hearing.  Please see Application for Development Review for items which require Public Hearing.  To learn more about the application process and procedures call  847-499-9063.
Application for Development Review	The standard application for all actions that require a Public Hearing. (For PUD Application see the next form below)  For information call 847-499-9063.
Application for PUD (Planned Unit Development) Review	For more information call 847-499-9063.
Application for Subdivision	Petition form for all subdivisions. Please note that if a subdivision requires variations the Application For Development Review must also be completed.  Click here for more information on the subdivision review process.
Application for Building Permit	This Permit must be obtained from the Community Development Department.  Do not use this SAMPLE form.  It is here for informational purposes only.
Application for Health Permit	Return your completed application to the Village Health Officer at Community Development.
Application for Multi-Unit Building for Rental Property	Complete this form and return it to the Village Health Office along with the appropriate fee.
Application for Single Family/Individual Rental Property	Complete this form and return it to Community Development with appropriate payment.

Cook County Residential Demolition Application	The Cook County Department of Environmental Control requires you to apply for a demolition permit from their agency.  Learn more about other Community Development Department requirements here.

Garage Requirements
Permission to Install a Lawn Sprinkler in a Public Street	If part of your lawn sprinkler system will be located in the public street, you will need to complete this form.
Plumber Letter of Intent (Sample letter)	Required for all Permits which include plumbing work per Illinois Public Act 094-0132

Application for Real Estate Transfer Certificate	This certificate is required for anyone who sells or transfers real estate (Effective August 1, 2005)
Direct Pay - Fax Authorization (WHEELING DIRECT PAY)	Return this application along with a voided check to the Village of Wheeling 2 Community Boulevard, Wheeling, IL 60090 or fax a copy of these items to  (847) 459-9692 - - Please continue to make payments by check until your billing statement indicates “WHEELING DIRECT PAID.”
Hotel and Motel Operator's Tax Return	Statement of Tax Receipts under the Provision of Chapter 4.09 of the Municipal Code of the Village of Wheeling. Return one copy with your remittance to Finance Department, Village of Wheeling, 2 Community Blvd., Wheeling, IL 60090.  Please notify the Village of any change in ownership.
